Coming home after a hospital stay can be the moment things go wrong — a fall, a missed medication, a struggle managing alone that lands someone straight back in A&E. Our hospital to home support is designed to close that gap, with carers in place from the day you’re discharged.
We typically start with more frequent visits in the first week or two — helping with mobility, medication, meals, and personal care — then step support down gradually as confidence and independence return. We can usually arrange this at short notice to fit around a discharge date.
Is Hospital to Home Support Right for You?
Hospital to home support might be the right fit if:
- You or a relative are due to be discharged from hospital and worried about managing alone straight away
- A hospital stay has left you weaker or less steady on your feet than before you went in
- You want extra support in place for the first few weeks to reduce the risk of a fall or readmission
- You need help managing new medication, dressings, or a changed daily routine after treatment
- Family live too far away, or can’t be there full-time, to help with the first stretch of recovery
